Ninja báo thù được xem là phần tiếp theo của bộ phim Ninja (2009). Phim mở đầu với hình ảnh bình yên trong cuộc sống của Casey Bowman (Scott Adkins). Anh đang là chủ nhân của một võ đường nổi tiếng và vợ anh, Namiko (Mika Hijii), lại đang mang thai đứa con đầu lòng của hai người. Thế nhưng một ngày nọ, Casey trở về nhà và phát hiện Namiko đã bị ai đó sát hại một cách dã man. Đau khổ và tuyệt vọng, anh điên cuồng tìm cách báo thù. Với sự trợ giúp của người bạn thân Nakabara (Kane Kosugi), Casey từ Nhật Bản đã bay đến Thái Lan, tìm tới những cánh rừng ở Rangoon để truy lùng hung thủ. Tại đây, anh xác định được kẻ thù là tên trùm ma túy xấu xa Goro (Shun Sugata)...
